造血幹細胞移植後合併症の考え方

　造血幹細胞移植(stem cell transplantation，SCT)は，ドナー造血幹細胞を移植し，造血，免疫系の再構築を図る治療法で，生着のためには宿主免疫を強力に抑制する必要があり，前処置に大量化学療法や全身放射線照射が行われてきた．シクロスポリンAやタクロリムス(FK506)のような極めて有効な免疫抑制薬の開発により，安定した成功が得られるようになり，前処置も放射線照射などによる破壊的処置だけでなく，非破壊的処置によるSCTも治療に応用されるようになってきた．造血幹細胞ソースも，非血縁者，末梢血幹細胞，臍帯血などと広く求めることが可能となり，血液疾患の標準的治療法として広く定着し，長期生存が可能となってきた．移植成績を左右するのは移植後合併症と原病の再発である．

　SCT後はさまざまな独特の病態を呈する合併症が発症する．動物を用いた移植実験の過程で明らかとなった移植免疫学の考え方をもとに，移植片対宿主病(graft versus host disease，GVHD)が臨床的症候群として提唱された．GVHDは極めて重篤な合併症と考えられ，SCTの成功への歴史はGVHD克服の歴史でもあったといえる．また，強い免疫不全状態に合併する日和見感染症への対応も重要であり，細菌，真菌，ウイルスと次々と有効な治療薬が開発され，予防投与も研究されてきた．GVHDの臨床病態とみなされてきた合併症のなかから，強力な前処置で生じる副作用である前処置関連毒性(regimen related toxicity，RRT)¹⁾，肝静脈閉塞症(veno-occlusive disease，VOD)²⁾，閉塞性細気管支炎³⁾，血栓性細小血管障害(thrombotic microangiopathy，TMA)⁴⁾などが特殊な移植関連合併症として明らかにされてきた．これらの多くは，標的臓器がGVHDと同一臓器にあり，組織学的所見の近似性から，報告以前は多くがGVHDに包括されてきた疾患である．
