血管内皮機能検査

血管内皮機能障害は，動脈硬化症，高血圧症，心不全などの種々の心血管疾患および関連する病態で観察されるが，特に動脈硬化の初期変化として捉えられ，心血管系障害のサロゲートマーカーと考えられている．血管内皮機能評価は循環器疾患の血管機能異常評価法の1つとして重要な検査法に位置付けられ，超音波装置による血流依存性血管拡張反応（flow mediated dilatation，FMD）測定が頻用されている．FMDは現在，半自動型血管内皮機能解析装置が臨床応用され，エコートラッキングや専用のデバイスも工夫され，簡便性，解像度，精度ともに向上している．最近では，指尖の動脈拍動の血流変化（reactive hyperemia peripheral arterial tonometry，RH-PAT）の測定が広く行われるようになった．しかし，欧米では血管機能検査のガイドラインがあるものの，心血管疾患予防の有益な指標とは位置付けられていない．いずれの指標も動脈硬化の早期発見に有用で，予後予測などに使用される可能性があり，今後のさらなる検討が望まれる．
