甲状腺髄様癌

甲状腺髄様癌（medullary carcinoma）はカルシトニンを産生するC細胞（傍濾胞細胞）を由来とする悪性腫瘍であり，わが国では甲状腺癌全体の約1～2％を占める比較的まれな腫瘍である．髄様癌細胞はカルシトニンや癌胎児性抗原（carcinoembryonic antigen, CEA）を分泌し，しばしば腫瘍内にアミロイドが沈着すること，多彩な病型からなる遺伝性髄様癌が存在することなど，濾胞上皮由来の甲状腺腫瘍とは異なる病態病理，臨床所見を呈する．診断には触診，超音波検査に加えて，穿刺吸引細胞診，血中のカルシトニンやCEAの測定が行われる．治療は手術療法が第1選択であるが，術前の治療方針の決定にはRET遺伝子検査が重要となっている．
