真菌の薬剤感受性検査

新しい知見

MEC（minimum effective concentration）：真菌の薬剤感受性検査は，カンジダなどの酵母を対象とした酵母用感受性検査と，アスペルギルスなどの糸状菌用感受性検査に二大別される．いずれも微量液体希釈法が主な検査法で，ほとんどは最小発育阻止濃度（minimum inhibitory concentration，MIC）を測定するが，新しい抗真菌薬であるキャンディン系薬の糸状菌に対する感受性検査では，MECを測定するという新規の判定法が2008年に追加された^1）．

ブレークポイント：カンジダに対する抗真菌薬のブレークポイント（break point，BP）〔BP：MIC値の臨床的意義（感受性（susceptible，S），中等度耐性（intermediate，I），耐性（resistant，R）などを示唆するMIC区分点〕は徐々に追加されており，2008年発行の，米国臨床検査標準化委員会（The Clinical and Laboratory Standards Institute，CLSI）の検査マニュアル，M27-A3のsupplement（M27-S3）では7薬剤のBPが提示されている．糸状菌に対するBPは未定である．
