ハイド症候群

Point

●大動脈弁狭窄症に，消化管血管異形成（angiodysplasia）に伴う消化管出血を合併したものを，ハイド（Heyde）症候群と呼ぶ．

●大動脈弁狭窄部位で生じる高いずり応力（shear stress）によって血液中のフォン・ヴィレブランド因子（VWF）高分子マルチマーが減少し，消化管をはじめとする各部位に血管異形成を生じると考えられている．

●消化管出血部位に対する外科的あるいは内視鏡的止血療法の効果は一時的で，VWF補充療法による止血効果も十分ではない．大動脈弁置換による原因除去が有効である．
