大腸癌の再発予知の最前線

はじめに

　大腸癌における微小リンパ節転移の意義については古くから議論されているが，最終的な結論はいまだ出ていない．遠隔転移がなく，組織学的検索（病理診断）によってリンパ節転移がないと判断されたpN0大腸癌は，手術治療のみで治癒が期待されるが，実際には一定の頻度で肺，肝などへの血行性再発をきたす．これは，リンパ行性経路を介さず，直接血管内に細胞が侵入した結果であると説明することも可能であるが，一方で，1枚の病理診断用のHE（hematoxylin-eosin）標本では診断できなかった微小転移が存在し，これが再発に関与するという説も根強い．
