糞便移植療法

糞便移植療法の目的

　糞便移植療法とは，いわゆる便移植で，他人の糞便を患者の腸内に移植する方法である．その目的は，健常人の腸内細菌を患者の腸管内に移植することで自身の腸内細菌を変化させ，疾病を治癒あるいは体質を改善させることを目的としたものであるが，その背景にはミクロビオームに関する研究の進歩がある．

　ヒトの腸管内には便1g当たり10¹²個の細菌数が常在し，それらは約500種類の属に分類される多用な細菌群から構成されている．正常な細菌叢がいったん乱れると，正常化するのに数日から数週間を要するとされている^1）．そこで，乱れた細菌叢を，正常な細菌叢を有するヒトの便で置き換えるというのが糞便移植療法の目的である．糞便移植療法の対象疾患として最も研究が進んでいる感染症はClostridium difficile感染症（C. difficile infection：CDI）である．その他に，潰瘍性大腸炎，クローン病，過敏性腸症候群，便秘症，肥満やメタボリックシンドロームのヒトに健康な痩身者からの便が移植されるといった研究が行われており，報告数は年々増加している^2）．
