3　溶血性尿毒症症候群（HUS）

　溶血性尿毒症症候群（hemolytic uremic syndrome：HUS）は1955年にGasserによって報告された疾患であり，①微小血管症性溶血性貧血，②血小板減少，③急性腎障害（acute kidney injury：AKI）を3主徴とする症候群である^3）．重症例では発熱と中枢神経症を合併する場合もあり，TTP（thrombotic thrombocytopenic purpura）との鑑別が困難である．

　HUSは臨床経過から，志賀毒素（shigatoxin：Stx）産生菌による下痢症を伴う典型的HUSと，Stxによる下痢症を伴わない非典型HUS（atypical hemolytic uremic syndrome：aHUS）に分けられる．典型的HUSは腸管出血性大腸菌（enterohemorrhagic Escherichia coli：EHEC）の感染（O157，O111，O26など）が原因となり，小児HUSの約90％を占め，予後は良好である^4）．一方，aHUSは小児HUSの5〜10％を占め，典型的HUSと比べて予後は不良である．
