痛風

Point

●痛風とは，関節内に形成された尿酸塩結晶に起因する関節炎を主徴とする疾患である．主要症状は母趾中足趾節（MTP）関節などに生じる急性関節炎であり，痛風発作と称される．痛風発作は間欠性であるが，その基礎病態である尿酸塩結晶の沈着は持続性であることが重要である．

●痛風はわが国で増加している．

●欧米の疫学調査で，痛風は心血管疾患，脳血管疾患，2型糖尿病，慢性腎臓病，尿路結石を発症するリスクが高いことが示されている．

●痛風特有の検査として，偏光顕微鏡を用いた尿酸塩結晶の証明がある．診断に大きな威力を発揮する．

●痛風の治療には，痛風関節炎の治療，痛風発作の予防，長期にわたる高尿酸血症の是正の3つの目標がある．生活指導は基礎療法として重要である．
