凝固波形解析の有用性

はじめに

　現在，臨床の現場であるベッドサイドや手術室で，凝固機能は全血を用いたPOC（point-of-care）装置で測定し迅速に評価されている^1）．古典的な全血を用いた凝固機能評価方法として，トロンボエラストグラフィ（thromboelastography：TEG）は有名だが，検体の保存や測定の再現性などが問題であった．凝固波形解析（clot waveform analysis：CWA）は血漿を用い，自動凝固分析装置によって各種凝固異常症の凝固機能評価が可能であり最近注目されている^2）．これらの方法の基礎や有用性について概説したい．
