—step up編—単球貪食試験による不規則抗体の評価とその臨床的意義

Point

●赤血球に対する不規則抗体が感作した赤血球の破壊は，脾臓でのマクロファージなどの単核食細胞による貪食（血管外溶血）が主たる要因である．

●貪食率が高くなる重要な要素は，赤血球に感作する抗体量であり，抗体価が低いまたは赤血球との親和性が低い抗体では貪食率が低い．

●生体内で自己抗体が感作した直接抗グロブリン試験（DAT）陽性赤血球は，抗体感作量と補体の関与により貪食（破壊）の程度は異なる．
