心不全患者におけるL波の発現様式とその意義

はじめに

　心エコー図検査でパルスドプラ法を用いた左室流入血流速波形は，心不全診療において重要な評価項目である．通常は拡張早期のE波と心房収縮期のA波の二相性であるが，拡張中期，E波とA波の間に順行性のL波を認める症例がある（図1）．正常例でも徐脈の際にL波を認めることがあるが，心不全患者におけるL波の意義と発現様式について述べる．
