心エコー検査時に肺高血圧を疑うコツ

はじめに

　肺高血圧症（pulmonary hypertension：PH）は種々の原因により肺血管抵抗，肺動脈圧が上昇する病態である．わが国のガイドライン^1）では，安静時に右心カテーテル検査による平均肺動脈圧が25mmHg以上の場合にPHと定義されている（今後20mmHgに引き下げられる可能性あり）．本稿では，症状および心電図などの各種検査からPHが疑われて依頼された際の心エコー検査のコツについて解説する．なお，心エコー検査の実際は，米国心エコー図学会のガイドライン^2）に標準的な指針が掲載されている．英文ではあるが図および図説だけでも一読してほしい．
