頸動脈❶ 総頸動脈解離

総頸動脈解離の原因には，外傷や特発性に発症する頸動脈原発性解離があるが，そのほとんどが急性発症する大動脈解離による波及である．中でも上行大動脈に解離腔を有するStanford A型解離により右総頸動脈に発症する場合が多く，malperfusion（分枝灌流障害）となり死亡に至るケースが多い^1）．ただし，波及の程度が大きい場合では両側総頸動脈解離となる場合がある．急性大動脈解離は激しい胸背部痛を伴うが，胸痛のみられない例が約10％存在する^2）．さらに，脳神経症状を呈する例は20％程度あり^3），両方が合わさった場合では，最初に脳塞栓などの脳神経疾患が疑われることとなる．急性脳梗塞の内科的治療として組織型プラスミノゲンアクチベーター（tissue-type plasminogen activator：t-PA）静注療法があるが，急性大動脈解離ではt-PA静注療法は禁忌であるため診断には注意が必要である．
