痛風

Point

●痛風とは，尿酸塩結晶が慢性的に関節内に沈着する疾患である．この尿酸塩結晶を原因として痛風発作が生じる．痛風発作は急性単関節炎であり，母趾中足趾節（MTP）関節に好発する．痛風はメタボリックシンドロームの合併が多い．

●高尿酸血症は，血清尿酸値7.0mg/dLを超える場合をいう．高尿酸血症は痛風の生化学的基盤である．

●痛風発作のときには血清尿酸値が低下することがあり，診断で注意を要する．この機序には，インターロイキン（IL）-6やコルチゾールが関与していると考えられている．
