悪心・嘔吐

概要

●悪心・嘔吐の病態，原因疾患

　悪心は“何かを吐きそうな感じ”，嘔吐は“口から唾液や消化液，吐物を吐き出してしまう状態”で，何らかの原因により嘔吐中枢が刺激されることで起こる．その原因は消化管閉塞や腹腔内の占拠性病変のみならず，自律神経障害，中枢神経疾患や電解質・ホルモンの異常など，多岐にわたる．領域別の疾患・病態としては，消化器領域以外に，腎・泌尿器領域，産科・婦人科領域，頭蓋内病変や薬物・中毒などが含まれる^1）．

　POCUSでこれら全ての疾患・病態を網羅することは容易ではないため，比較的頻回に遭遇し，臨床的に重要かつ典型例で，検出が可能な疾患あるいは病態についての評価を目指すことになる．具体的には，急性胆囊炎および胆石発作，尿管結石，消化管穿孔，腸閉塞，急性虫垂炎，大動脈瘤および大動脈解離，腹水貯留などが挙げられている^2）．
