尿ナトリウム／カリウム比と血圧の関係—臨床的意義と最新知見

はじめに

　高血圧は，日本人の死因の上位を占める心血管疾患（cardiovascular disease：CVD）（脳卒中，心筋梗塞など）の主要な危険因子である．その予防と管理には生活習慣の改善が重要であり，なかでも食塩（ナトリウム）摂取の制限とカリウム摂取の増加は，極めて効果的な戦略とされている^1）．これらの摂取バランスを簡便に評価できる指標として，尿中ナトリウム／カリウム比（以下，尿Na/K比）が注目されている．尿Na/K比は，食事におけるNaとKaの摂取量を反映し，血圧の状態や心血管リスクと密接に関連している^2）．

　従来，食塩摂取量の評価には24時間尿中Na排泄量が用いられてきたが，煩雑で継続的な測定には不向きである．一方，尿Na/K比は，随時尿からも算出可能であり，日常的なモニタリングにも適している．

　本稿では，尿Na/K比の基礎的知識，測定方法，血圧との関連性に加え，最新のガイドラインや他疾患における応用について詳述する．
