ウイルス肝炎の自然史と発がん

肝がんの主要病理組織型は肝細胞がん(以下HCC)と胆管細胞がん(以下CCC)の2種類である．大阪府がん登録(1993～97年罹患患者)によれば，病理組織型が判明している肝がんの内，男では前者が90％，後者が9％，女では各々82％，16％を占めた．わが国のHCCは，B型肝炎ウイルス(以下HBV)またはC型肝炎ウイルス(以下HCV)の30～40年以上の持続感染を経た後，慢性肝炎，肝硬変に併発するものが大多数である．

　本稿では，わが国における肝がん発生の動向とその特徴について述べるとともに，肝炎ウイルス感染との関連，肝炎ウイルス感染における肝発がん発症までの自然史について述べる．また肝がん予防対策として，肝炎ウイルスのスクリーニングをどこまで行っていくべきかについて考察する．
