　歯周病は，歯と歯肉の境目にある歯周ポケットの慢性嫌気性菌感染症である．歯周ポケットには数多くの嫌気性菌が相互に連絡しあって構成する粘着性のバイオフィルムがあり，生体内の免疫から巧みに自己を守っている．歯周病局所におけるバイオフィルムとそれを構成する嫌気性菌は，近年，心疾患，糖尿病，低体重児出産などとの関連が指摘されるようになってきた．

　歯周病菌が生体に影響を及ぼす過程としては，細菌自体が生体内に侵入する場合と，細菌の産生する物質(内毒素，サイトカイン)が入り込む場合がある．バイオフィルム内には大量の細菌が生息しており，何らかのきっかけによって，生体内へ侵入し，それぞれに適した部位に到達すると疾患を惹起することが可能である．しかし，実際に「病巣感染」と呼ばれたこれらの遠隔部位における疾患のメカニズムが明らかになってきたのは，分子生物学的手法の進歩が著しいごく最近のことである．

　本稿では，心疾患に関連した話題として，「感染性心内膜炎」と「冠動脈疾患」について，歯周病細菌との関連を中心に記載する．
