米国医療制度の概況―医療制度分析の視点から

　わが国において医療制度改革への関心が高まるにつれ，米国の医療制度が引き合いに出される機会が増えている．ただ，米国の医療制度については，先進医療技術や医療職の教育システムなどの光の面と，無保険者に代表される影の面が混在し，それらが別個に議論される傾向があることから，文脈によって得られる印象が異なり，その全体像の理解が困難となっている．

　筆者は2005～2007年にかけて，米国ハーバード大学において医療政策に関する一連の講座を受講した^注1)．本稿ではそこで用いられていた分析枠組みの一部を用いて，米国の医療制度の成り立ちを概観することで，その全体像の理解への手がかりの提供を試みたい．
