子どもにおけるインフルエンザワクチンの個人予防効果を検証する

はじめに

　わが国ではかつて，学校など集団生活でのインフルエンザ蔓延が地域社会への感染拡大につながるという考え方から，1962年から勧奨接種として，1976年からは予防接種法のもとに学童中心に集団接種が行われていた．ところが，その有効性，流行抑制効果および安全性が明確でないという議論がなされるようになり，1994年の予防接種法改正によってインフルエンザワクチン接種が定期接種から任意接種に変更されたこともあり，予防接種に対する社会の関心は著しく遠のいた時期があった．しかし，高齢者やハイリスクグループに対するワクチン接種の有効性については世界的に数多くの報告がなされており，その後，わが国において高齢者のインフルエンザ関連死が急増したこともあって，2001年の予防接種法改正で高齢者などを対象に，インフルエンザワクチン接種が行われることとなった．これにより，わが国のインフルエンザ対策は，やっと世界レベルに追いつきつつある．

　近年，わが国でも予防接種への関心が高まり，過度に有効性を示す報告が増加している．しかし，この間に研究デザインや解析方法に大きな進歩があったとは必ずしも認めることができず，明らかにpublication biasの存在が窺われる．

　このような観点から，本稿では，インフルエンザワクチンの予防効果を「有効性」と「適応」に焦点を当てて概説する．
