③胃がん予防策としてのヘリコバクター・ピロリ除菌療法の展望

　ヘリコバクター・ピロリ(H. pylori)は生涯に亘って胃粘膜に感染して胃炎を惹起する．慢性胃炎を背景として胃・十二指腸潰瘍，胃がんなどの様々な上部消化管疾患が起きる．胃がんは組織型を問わずH. pylori感染粘膜から発生することがほとんどである．高度の萎縮，腸上皮化生，体部胃炎では分化型胃がんが，非萎縮や全体胃炎では未分化型がんの発生が高く，背景粘膜の炎症や萎縮の程度によって，胃がんリスクが異なる．動物実験ではH. pylori除菌が胃がん発生を抑制することが認められ，除菌を早期に行うほうが胃がん予防の効果は強い．中国でのH. pylori除菌による無作為二重盲検比較試験では，全体では除菌は胃がん発症を抑制しないとの結果であった．しかし，胃がんの内視鏡的切除後の異時性多発がんをエンドポイントとした無作為化試験がわが国で行われ，H. pylori除菌によって有意に異時がんの発症が抑制された．H. pylori除菌は胃がんの発育進展を抑制すると推測できる．胃がん撲滅のために，H. pylori除菌を基本に据えた施策を早急に計画すべき時期に入った．
