公衆衛生制度の基本構造

　リバプールで，1848年にWilliam Duncanがはじめて保健医官(Medical Officer of Health:MOH)に就任し，生活環境改善などの予防的な公衆衛生施策の実施により貧困地域における乳幼児の死亡率を大幅に改善する著名な功績を上げたことから，1871年に自治体に保健医官を置くことが制度化されるようになった．1875年の公衆衛生法改正により全国の自治体に波及していった．公衆衛生の担い手は「自治体」とし，「保健医官」が存在する形が整えられた．この制度の特徴は，「自治体」とそこに「専門職」を位置づけたということにある．これ以後，自治体の中の専門職を専門職らしくどのように育て，仕事をさせていくかが，公衆衛生対策の重要な課題として今日まで大きな命題として存在し続けている．
