公衆衛生政策における現在知の集積・総合・共有―英国からの示唆

はじめに

　社会におけるコミュニケーションが発達し，人々の健康に関する複雑な要因が明らかになる中で，また公共政策の課題として規制や給付が行われる中で，公衆衛生政策の根拠となる知識の集積・総合・共有をどのように行うかは，重要な探求課題となってきている．本稿では，公衆衛生政策の形成に関わる知識の集積・総合・共有のあり方について，90年代末からさまざまな新しい展開を試みてきた英国の，公衆衛生政策の展開から示唆を引き出してみたい¹⁾．

　与えられた紙幅を考慮し，英国の公衆衛生政策とその実施過程に関わる要点を説明した後，以下の3点に限定して論述する．まず，公衆衛生政策の目標に関わって，人々の健康の全体的な向上と健康の格差の両方を明示的に課題としている点，政府の実績評価の一部に位置付けられている点などを述べる．次に，健康情報の把握と活用，公衆衛生政策の社会的議論のあり方，特に実践的知識の分析・収集・活用に向けて，公衆衛生観測機関を紹介・検討する．最後に，公衆衛生政策の理論と根拠について，明確にまた総合的に提示していくことについて考える．

　70年代半ば以降，英国の公衆衛生制度は国民保健サービス(National Health Service:NHS)に組み込まれており，「人々の健康に関する集合行為(collective action)」²⁾としての公衆衛生のあり方は日本とかなり異なっている．また実際の組織や制度はしばしば変更される．その点を踏まえて，学びうることは何かに関心を払いながら議論を進めたい．なお，NHSを含む保健・医療政策の概況については，重要な資料が先頃翻訳されたので，そちらを参照されたい³⁾．
