肝疾患と糖尿病の最前線

はじめに

　肝臓は，糖代謝の中心的役割を担っており，その障害は糖代謝異常の発病に密接に関連している．また，肝発癌は，慢性肝障害の終末像であり，糖代謝異常が合併する硬変期において高頻度に見られる．肝硬変において二次的に発症する糖尿病は，肝性糖尿病と言われている．また，肝硬変・肝癌の主な原因であるC型肝炎や内臓脂肪蓄積による中心性肥満に伴う脂肪性肝炎の患者には，しばしばインスリン抵抗性が見られ，糖尿病を合併する割合が高いことが知られている．最近の疫学研究によれば，糖尿病は，肝発癌の重要なリスク要因であることが示されている．したがって，患者の耐糖能異常と肝発癌へと至る慢性肝臓病の進行には関連性が示唆される．本稿では，肝疾患と糖尿病の最近の知見について述べる．
