　公衆衛生活動の現場では，原因が複雑で，有効な解決方法が見当たらず，対処に窮する課題に出会うことは少なくない．例えば，次のような問題を考えてみよう．平成19(2007)年の国民健康・栄養調査によれば，医師から糖尿病と言われたことがある者のうち未治療者は4割に上ると推計されている．なぜ，かくも多数の患者が治療を受けていないのであろうか．経済的負担や仕事が多忙という理由の他に，自己管理で大丈夫と考える傾向や，医師に生活不節制を指摘されることで心理的ストレスを感じることなども一因になっていると考えられる．仮に，こうした治療の阻害要因を十分に把握したとしても，公衆衛生的に有効な対策を講じることに直結しないこともしばしば経験する．

　このように，公衆衛生の活動には痒いところに手が届かないような難題が存在するのである．気になりつつも対策の糸口が見つけられないままに放置されている課題が少なくないと思われる．
