CKDの早期発見の重要性

　慢性腎臓病(以下，CKD)は，末期腎不全(以下，ESKD)に至る患者を減らすとともに，腎臓病患者に合併しやすい心血管疾患(CVD)を防ぐことを目的として，2002年にアメリカの腎臓財団により作られた概念である．CKDの定義は，医師，看護師，薬剤師，栄養士などの医療関係者だけでなく，一般住民にもCKDの重要性とリスクを説明できるように，蛋白尿に代表される腎障害の存在，または糸球体濾過量(glomerular filtration rate:GFR)60ml/min/1.73m²未満と単純なものに決められた¹⁾．さらにCKDは早期発見により，治療可能な疾患であり，適切な血圧管理と尿蛋白に対するレニン・アンジオテンシン系阻害薬による治療が確立している．

　このシンプルさと分かりやすさから，わが国においても医学界でCKDの概念はすぐに受け入れられ，短期間に広まった．わが国のCKD患者数は1,330万人であり，成人の13%を占め²⁾，新たに見出された21世紀の国民病であることも判明した．CKDは30万人を超す維持透析患者の予備軍であるばかりでなく，心血管疾患発症の最も強いリスクファクターであることから，人類に対する新しい脅威として再認識された．
