イギリスにおける公衆衛生の歩みと新たな展開―パブリックヘルス・イングランド

はじめに

　イギリスの公衆衛生を理解するのは容易ではない．その難しさはイギリスが単純に二分法で割り切ることはできない社会であることにある．宗教改革，市民革命，産業革命などの歴史の中で社会制度がつくられてきたからと思われる．イギリスは資本主義経済を確立し，その先頭を走ってきた．そのためにその負の社会問題にいち早く直面してきた．海外進出，植民地の抑圧，その反動として独立運動，そして妥協と協調により今日の社会に至っている．

　多くの人々の血が流され，多くの貧困・困窮者，多くの病人・死亡者を発生させてきた．それらの社会矛盾を克服する知恵として，救貧法，工場法，公衆衛生法，住宅法など，多くの人々を守る新たな社会制度を誕生させてきた．その最大のものが第二次世界大戦後，荒廃し，疲弊した時代の中にありながら，すべての人々に対し無料で医療サービスを提供する国民保健サービス(National Health Service;NHS)である．今日も堅持されている．

　19世紀にイギリスが誕生させた公衆衛生制度は戦後NHSがつくられたことにより，停滞を余儀なくされた．しかし，NHSのかたちが整った1980年代後半から公衆衛生制度に対する関心が高まり¹⁾，その後，プライマリケア・トラスト(Primary Care Trust;PCT)，ヘルスプロテクション・エージェンシー(Health Protection Agency;HPA)がつくられ，2013年4月にパブリックヘルス・イングランド(Public Health England;PHE)としてNHSとは独立した体制の確立に至った．

　近代社会における「公衆衛生」とは何かについては，イギリスが常に新しい考え方を提示してきている．そこで，パブリックヘルス・イングランドに至るまでのイギリスがつくりあげてきた公衆衛生を鳥瞰する．
