途上国におけるNCDの現状

1．健康の定義とNCD

　世界保健機関(world health organization;WHO)の健康の定義を変えようという動きがある．2011年，英国医学雑誌(BMJ)上で，Huberら¹⁾が議論し，「健康とは，社会的，身体的，感情的な困難に直面した時，それに適応し自己管理できる能力」としたらどうか，と提言している．その根拠の1つがnon-communicable disease(NCD)の蔓延である．

　長らく君臨してきたWHOの健康の定義が世に示されたのは1948年．世界中どこもかしこも結核をはじめとする感染症が最大死因を占めていた時代であった．ところが半世紀以上が過ぎた2008年，世界の死亡者数5,700万人のうち，約3分の2にあたる3,600万人がNCDで死亡している²⁾．とりわけNCDの4大疾患である心血管疾患，糖尿病，がん，慢性呼吸器疾患が主要死因である．いずれもかつては先進国の代表的死因であったもの．それが，今や途上国でも主要死因となっている．しかもNCD関連死亡の70％は途上国からである³⁾．高齢化，都市化，危険因子のグローバルな広がりによって，NCDを抱える人は途上国でいっそう増えるであろう⁴⁾．このようなNCD増加という状況下で，これに適応し自己管理できる能力を「健康」とする，という定義の提言は，少なくとも成人においては理にかなっている．
