COPD患者の自然経過と治療効果

　慢性閉塞性肺疾患(chronic obstructive pulmonary disease;COPD)は，通常緩徐に進行性の疾患である．「COPDの進行性」とは病理変化で言えば肺胞破壊(気腫化)および気管支内腔の狭小化が進むことであり，臨床上は呼吸器の検査での閉塞性障害の指標である一秒量(FEV₁)の低下で判断される^1,2)．
　近年，わが国はもとより世界的に，COPDは予防と治療が可能な疾患とみなされるようになってきた^1,2)．その論拠として，COPD治療薬の改良，具体的には薬効の長時間作用性化，吸入デバイスの改良等が挙げられる．加えて，COPDに対する薬物介入の大規模試験で，薬剤の症状改善効果はもちろん，疾患進行や増悪頻度といったCOPDの自然歴に影響を与え，死亡率まで低下させる可能性が示唆された点が挙げられる．
