アトピー性皮膚炎の遺伝要因と環境要因

　フィラグリンは，角質の形成に重要なタンパク質で，皮膚バリア機能の維持と保湿に重要な働きをしている．皮膚バリア機能とは，皮膚からの水分喪失阻止や，外界からの異物の侵入阻止などの機能をいう．2006年にフィラグリン遺伝子変異がアトピー性皮膚炎の重要な発症因子であることが報告された．現在では，アトピー性皮膚炎の多くは，フィラグリン遺伝子変異などの遺伝要因と，種々の環境要因による皮膚バリア機能障害を基盤として発症するバリア病と考えられている．本症の病態と治療を考える上で，皮膚バリア機能の破綻に伴う慢性抗原刺激がアトピー性皮膚炎の本態であると理解することが極めて大切であり，皮膚バリア機能の是正がアトピー疾患の改善や予防に役立つ．
