がん検診の拡大に伴う過剰診断等の問題点と今後の対策

がん検診の利益と不利益
　がん対策の目的の1つは，がん死亡を減少させることにある．がん検診は，がんを無症状のうちに早期発見することで，この目的を達成させる主要な手段の1つである．一方，がん検診を行うことで，一部の受診者に不利益をもたらすことが認識されてきた．がん対策としてがん検診を導入する場合，がん死亡減少という利益と下記に挙げる不利益とのバランスを評価し，利益が不利益を上回る場合に限って導入することが求められる．
　がん検診の不利益としては，検診に伴って間違って判断される偽陰性(がんがあるのに検診で陰性となる場合)，偽陽性(がんがないのに検診で陽性となる場合)，がん検診そのものがもたらす偶発症がある．さらに，こうした間違った判断の結果ではなく，すべての判断が正しく行われた結果もたらされる不利益として，過剰診断の存在が認識されるようになってきた．
