糖尿病網膜症の現状と対策

はじめに
　糖尿病を起因とした眼疾患は，糖尿病網膜症(以下，網膜症)に限られたものではなく，眼瞼・角膜異常，屈折・調節障害，白内障，虹彩・ぶどう膜炎，緑内障，虚血性視神経症，外眼筋障害の多岐にわたる．しかし，その有病率と視力予後において網膜症が重要であり，糖尿病の内科的管理および網膜症の眼科的治療が進歩しているなか，いまだ網膜症は世界の働く世代における失明の主たる原因疾患となっている．一方，網膜症の一病態に，中等度の視力障害を生ずる糖尿病黄斑浮腫がある．人間の網膜には黄斑という視力を司る重要な部分があり，それが機能することによって中心視力が得られる．網膜症は，網膜全体に病変が現れるが，黄斑に病変が出現すると中心視力に直撃するため視力低下の大きな原因になる．糖尿病黄斑浮腫は，黄斑部の血管透過性亢進による黄斑部に網膜浮腫や硬性白斑が沈着する病態を言う．最近，糖尿病黄斑浮腫に対して，抗血管内皮増殖因子(vascular endothelial growth factor;VEGF)薬の硝子体注射が新たな治療として適応となっている．
