英国の地方自治体における公衆衛生—総合性と専門性，リーダーシップ性

自治体を基盤とした公衆衛生とリーダーシップの確立
　古代ギリシア(紀元前400年ごろ)において，すでに健康を清潔な水，清い空気，適切な住居などの環境とのホリスティック(holistic)なものとする捉え方があった．しかし，社会的に制御する制度はつくられていなかった¹⁾．14世紀ごろになると，ペストの脅威に対して，北イタリアのフィレンツェなどの諸都市では衛生対策が講じられていた²⁾．18世紀のオーストリアでは，国民の人口政策に国家が介入を試みた．しかし，自治体を基盤とした近代的な公衆衛生制度は英国において19世紀に確立されたものであった．
　英国では，18世紀末に，ジェレミー・ベンサム(Jeremy Bentham)は少数者の利益を図る社会から多数の人々の幸福を考えた社会(the greatest happiness of the greatest number)への転換を促す政治思想を普及させた³⁾．1835年の都市団体法の成立などを基にして，近代の自治体が形づくられてきた⁴⁾．そしてエドウィン・チャドウィック卿(Sir Edwin Chadwick)は自治体を土台として生活衛生対策を講じることを具現化した¹⁾．自治体に保健医官(Medical Officer of Health:MOH)，衛生監視員(Environmental Officer of Health:EHO)，食品分析検査員，土木技術者などを置くことを求めた．最初のMOHはリバプール市に1847年に着任したウイリアム・ダンカン(William Duncan)であった．ジョン・シモン(John Simon)は1948年にロンドンの初代MOHとなり¹⁾，その後，中央保健局(General Board of Health)の初代首席医務監となった．保健医官のリーダーシップによって都市の住宅や環境衛生の状況が改善し，死亡率低下に成功した．
