高齢者の聴覚障害への支援—健康寿命を延ばすために

—なぜ高齢者の難聴は問題なのか？
高齢者の難聴は「ただ音が聞こえにくい」というだけの問題ではない．高齢者の難聴は健全な心身の維持に関わる問題であり，うつや社会的孤立の原因になる1)2)．また，難聴であることは認知機能低下のリスク要因となる3)4)．認知症のリスク因子を図13)に示す．このうち，難聴の影響は，改善し得る要因の中で一番大きく，難聴を解決すれば，世界中で9%の認知症患者を減少させることが期待できる．一方で，高血圧，肥満，喫煙，高血糖が難聴の発症要因になっている可能性があり5)〜11)，難聴にまつわるさまざまな問題に目を向けることは，認知症という高齢者における大きな課題への取り組みにつながる．
難聴の影響は脳機能のみならず身体全体の機能低下にまで影響を及ぼし，高齢者が「フレイル」(frailty)といわれる状態に陥るリスク要因となる12)13)．フレイルとは，加齢とともに臓器予備能が低下し，全身のストレス耐性と心身の活力(例えば筋力や認知機能など)が低下することで，生活機能障害，要介護状態，そして死亡などの危険性が高くなった状態のことである14)．75歳以上の高齢者における要介護の原因の1位がこのフレイルである15)．
その他にも，難聴を有することは，転倒や，何らかの疾患で入院を要するようになるといったさまざまな不利益につながることが報告されている16)．人口が減少傾向となり，2030年には総人口の1/3が65歳以上(2017年時点では27.7%)となる超高齢社会の日本において17)，いかにして高齢者が心身の健康を保ち，活発に学問，経済活動に参加するかは国力に関わる重大問題である．
