睡眠障害と糖尿病

はじめに
 最近20〜30年の睡眠不足の増加は公衆衛生学領域では深刻な問題と認知されている一方,社会では必ずしも十分に認識されていない.睡眠不足とは睡眠が十分にとれていない状態であり,睡眠が足りなくなる原因は,①睡眠時間が量的に不足する場合と,②睡眠呼吸障害(sleep disordered breathing:SDB)などによって睡眠の質が低下する場合とが考えられる.
 睡眠不足とともに糖尿病ならびに肥満も増加傾向にあることから,両者の関連が示唆されている1).疫学的,生理学的な研究によって,睡眠障害が糖尿病やメタボリックシンドロームの原因となり得ることが明らかになってきた1).そのメカニズムの一つとして,睡眠不足によるレプチン減少とグレリン増加が食欲を亢進させる働きが明らかにされている.また,睡眠不足による交感神経活動の亢進は,インスリン抵抗性の上昇および耐糖能低下を引き起こし,糖尿病のみならず心血管障害の原因ともなり得る2).睡眠不足を含む睡眠障害がもたらす代謝異常は成人だけでなく,若年あるいは児童の健康にも影響する.
 本稿では,睡眠障害と糖尿病に関する最近の知見を紹介し検討する.
