英国のプライマリケアデータベースの利活用

はじめに
　英国では，かかりつけ医であるgeneral practitioner(GP)によるプライマリケアと，専門医による診療や入院医療であるセカンダリケアが明確に分けられている．患者は緊急時を除いて，登録した診療所をまず受診し，必要に応じて専門医に紹介を受けるという形の医療提供体制(ゲートキーパー制)が整えられている．このことは，プライマリケアの診療記録を追跡することで，患者の病歴や治療をほぼ切れ目なく把握できることを意味している．診療所の診療情報は，定型化された電子健康記録(electronic health record:EHR)として保存されている．現在では，数百施設に及ぶ診療所のEHRが集約・データベース化され，多くの研究に活用されている．そこから得られた数々の「リアルワールドエビデンス」は公衆衛生上，重要な役割を担っている．
