日本における費用対効果評価の取り組み

はじめに
　わが国では，国民皆保険の下，有効性・安全性が確認された医療であって必要かつ適切なものは，薬価制度，材料価格制度に基づいて保険適用することを基本としている．しかしながら，急速な高齢化に加え，近年の高額な医薬品・医療機器の登場などによる医療保険財政への影響が懸念されている．今後も革新的で高額な医療技術の保険適用は増加すると考えられており，医療保険財政との両立をどのようにしていくかが課題となっている．
　医薬品・医療機器に対する費用対効果評価制度^1)〜3)については，2012年5月に中央社会保険医療協議会(中医協)に費用対効果評価専門部会が設置され，検討が開始された．その後，2016年の試行的導入⁴⁾⁵⁾を経て，2019年4月より制度運用が開始となった(図1)⁶⁾．費用対効果評価制度を円滑に導入するため，薬価・材料価格制度との整合性を取りつつ制度設計が行われた．
　本稿では，わが国における費用対効果評価制度の特徴および今後の展望について記載する．
