ポイント
◆わが国の高齢者結核のほとんどは、過去に感染したものの当時は発症せず、内因性に活性化する二次結核を特徴とする。
◆高齢者における市中肺炎と肺結核の鑑別は、臨床症状、検査所見、画像所見など非特異的であることから困難なことが多い。
◆高齢者の市中肺炎においては、結核をスクリーニングするために広く喀痰抗酸菌検査を行うことが望ましい。
