1．当該疾患の発生動向
　B型肝炎ウイルス（hepatitis B virus: HBV）は世界で約2億9,600万人が感染しており、毎年約82万人が肝硬変や肝がんで死亡している^1）。わが国のHBV感染率は約1%であり、HBVキャリア数は130〜150万人であると報告され重要である^2）。そのため急性B型肝炎は5類感染症に指定され、法律的には届出が必須である。
　5歳未満の乳幼児感染と成人感染では経過が異なる。図1^2）に示されるように、乳幼児期の感染は母子感染による垂直感染が主であり慢性化率が高い。一方、水平感染は主に成人の感染で経過中約90%においてHBs抗原は陰性化しHBs抗体、HBc抗体を獲得し臨床的寛解状態となる。一部が急性肝炎を発症し臨床的寛解に至る。
