COVID-19パンデミックの教訓から—国際保健規則（International Health Regulations）の改正

はじめに
　毎年5月、世界保健機関（World Health Organization: WHO）の総会が開催される。2024年の第77回WHO総会（World Health Assembly; WHA）（5月27日から6月1日に開催）では、COVID-19パンデミックの教訓を踏まえて、194カ国のWHO加盟国が約2年間交渉を行っていた国際保健規則（2005年）（International Health Regulations（2005）: IHR（2005））の改正案が採択された^1）。
　IHRとは、国際交通および取引に対する不要な阻害を回避しつつ、疾病の国際的伝播を最大限防止することを目的とした、WHO憲章第21条に基づいて採択された規則である。WHO加盟国と未加盟のリヒテンシュタインおよびバチカン市国の計196カ国（参加国、States Partiesと呼ばれる）が法的拘束下にあり、参加国は、IHRに規定されたさまざまな義務を果たす必要がある。例えば、第5条において、事象を検知・評価・通報・報告する能力を強化および維持すること、第13条において、公衆衛生リスクおよび国際的に懸念される公衆衛生上の緊急事態（public health emergencies of international concern: PHEIC）に迅速かつ効果的に対応すること、第19条において、空港・港や国境の衛生管理能力を備えておくことが義務付けられている^2）。さらに、IHRでは各国が健康危機に備え、対応するために整備すべき基本的能力である「コアキャパシティ」が規定されているが、COVID-19の流行下では、コアキャパシティを満たしている先進国も甚大な影響を受けた。これを踏まえ、IHRの改正に向けて、2021年のWHA75でIHR改正のための加盟国作業部会の設立が決定し、同年9月に日本を含む一部の参加国がIHRの改正案を提出した^3）4）。そして、2022年から改正案の交渉を開始した。
　本稿においては、IHRの歴史を振り返りつつ、COVID-19を契機に交渉が開始し本年採択に至ったIHR改正案について概説したい。
