B型肝炎ワクチン—小児を中心に

1．当該疾患の発生動向^1）
　一般に3歳以下の乳幼児のB型肝炎ウイルス（hepatitis B virus: HBV）感染の特徴は、年長児や成人とは異なりHBV感染により容易にキャリア化する点である。例えば、HBe抗原陽性のHBVキャリア妊婦から生まれる小児では、無処置の場合はほぼ100%にHBV感染が見られ、90%以上はキャリアとなる。図1^2）にHBVキャリア成立と年齢の関係を示した。HBV感染によりキャリア化するのは新生児、乳幼児が主体である。このことは大変重要な点であり、HBVキャリアを撲滅するためには小児期早期からの対策が不可欠である。
　HBVの持続感染による肝細胞癌（hepatocellular carcinoma: HCC）は、日本では部位別癌粗死亡率で男性第3位、女性第5位となっており、日本の肝細胞癌の約20%を占めている。HBVに関する治療薬は進歩したが、まだ完治することはできない。したがって、HBVの持続感染者（キャリア）を発生させないことが最も重要な対策となっている。
