閉塞隅角緑内障の手術適応

はじめに

　原発閉塞隅角緑内障は日本緑内障ガイドライン^1）で，相対瞳孔ブロックが関与するものとプラトー虹彩によるものに分類された。閉塞隅角緑内障は開放隅角緑内障とともに緑内障の2大病型の1つであり，その頻度は多治見スタディでは原発閉塞隅角緑内障を含めて有病率は40歳以上の1.3％と報告され^2），全緑内障患者の約20％に相当することも明らかにされた。本症はさらに人種差，地域差，性差（女性が圧倒的に多い）があることもその特徴として知られている。閉塞隅角緑内障は基本的には相対瞳孔ブロックの解除，プラトー虹彩形状の緩和，水晶体の処理など外科的な治療が優先される疾患である。

　本稿ではそれぞれの病態に対して現状で行われている外科的治療についてその種類およびその問題点について述べ，次いでそれぞれの手術手技の適応について最近の方向性を交えて述べる。
