未熟児網膜症―手術に踏み切るタイミング

はじめに

　未熟児網膜症（retinopathy of prematurity：ROP）は網膜の未熟性を基盤として起こる疾患である。

　Terry^1）による最初の報告以来，半世紀以上経過しているにもかかわらず，未だその発症に関するメカニズムは完全に解明されておらず，失明児を完全になくすに至っていない。そればかりか，近年の周産期医療の発達に伴いきわめて未熟な児でも生存可能となり，厚生省分類Ⅱ型に相当する重症未熟児網膜症症例が増加している。

　重症未熟児網膜症症例では，しっかりと光凝固を施行しても網膜剝離に進行する場合がある。いったん網膜剝離を発症した未熟児網膜症症例は，輪状締結術や硝子体手術を施行しても，術後視力はもちろん網膜復位率に関しても決して満足できる状況ではない^2～8）。

　このような状況を踏まえてか，2005年には未熟児網膜症の国際病期分類が改訂され^9），わが国で以前から厚生省分類Ⅱ型として定義されていた病態が「aggressive posterior ROP」として追加定義された。

　重症未熟児網膜症のさらなる増加が予想される現況において，本稿では，未熟児網膜症に対して適切な時期に適切な治療がなされるよう，そのタイミングと治療法について，特に近年提唱されている早期硝子体手術に重点をおいて述べる。
