両眼に網膜動脈分枝閉塞症が発症した1例

要約　目的：両眼に網膜動脈分枝閉塞症が発症した症例の報告。症例と経過：狭心症と脳梗塞の既往がある62歳男性が起床時に左眼霧視を自覚して来院した。矯正視力は右1.0，左0.1であった。左眼乳頭から黄斑に向かう網膜が白濁し，網膜動脈分枝閉塞症と診断した。プロスタグランジンE₁リポ化製剤の点滴を3日間行い，8日後に視力は0.5に改善した。初診から3週間後に網膜中心動脈閉塞症に発展し，視力が0.1以下になり回復しなかった。2年後に右眼の乳頭から黄斑下方に向かう部位に網膜動脈分枝閉塞症が発症した。矯正視力は1.0であった。プロスタグランジンE₁リポ化製剤の点滴を1か月行い，アスピリンとワーファリン内服を継続した。6か月後の右眼視力は良好であった。結論：網膜動脈分枝閉塞症に対して行うプロスタグランジンE₁リポ化製剤の点滴と抗凝固薬内服では，その投与期間に留意することが望ましい。
