はじめに

 感染症は原因が外界から来た微生物であることが明確であり,古くから,病因となっている微生物に対していわば「標的治療」をしてきたわけであるから,「どこまで進んだ」といわれても,「標的治療なんて昔からやっている」と簡単に答えてしまいそうである。しかし,感染が生じる機構や感染によって生じる炎症の「分子病態」については,微生物側の因子,ホスト側の因子,その他の環境などの因子の3つが複雑にからみ合っているために,他の疾患,例えばホスト側の因子のみで起こる遺伝子病に比べてその機構はきわめて複雑であり,「どこまで進んだ」といわれると,逆に「まだまだこれからであまり進んでいない」ということになる。まして,「分子病態に基づく標的治療」となると,「海のものとも山のものともつかない」という状況だが,ともあれ,本稿では分子病態の研究がさかんなヘルペス性角膜炎を題材に,分子病態と標的治療の最近の動向についてレビューしてみたい。
