眼付属器MALTリンパ腫の放射線治療成績

2005年までの8年間に眼付属器の粘膜関連リンパ組織（MALT）のリンパ腫13例を治療した。男5例，女8例で，7例が両眼性，6例が片眼性である。年齢は38～83歳（平均58歳）であった。8例では腫瘍は結膜に限局し，5例では眼窩結合組織原発の腫瘍であった。化学療法は行わず，全例に放射線治療を行った。線量は，病巣部局所の照射野で30～54Gyであった。4～92か月（平均53か月）の経過観察で，結膜のリンパ腫は完治し，球後リンパ腫のうち3例は完治，2例は改善した。眼窩内筋円錐の外方にあるリンパ腫は消失し，筋円錐の内部にある線状の腫瘍と内直筋を含むリンパ腫は放射線治療後も残存した。以上の事実は，眼窩のMALTリンパ腫では腫瘍の部位と性質により放射線照射に対する反応が異なることを示している。有害な晩期合併症は40Gy以上の照射で起こり，この量が眼窩MALTリンパ腫に対する放射線治療の上限であると推定される。
