中心窩下脈絡膜新生血管に対する経瞳孔温熱療法の治療成績

目的：中心窩下脈絡膜新生血管に対する経瞳孔温熱療法の効果を検討する。対象：中心窩下脈絡膜新生血管を有する患者25名25眼。狭義の加齢黄斑変性症15眼，ポリープ状脈絡膜血管症8眼，その他2眼で，年齢は51～88歳（平均74歳）であった。結果：治療前の平均視力は0.11であり，治療1か月後で0.11，3か月後で0.10，6か月後で0.08であった。治療前後で平均視力に有意差はなかった。3段階以上の視力改善は治療1か月後で1眼，3か月後で2眼，6か月後で1眼であり，不変はそれぞれ24眼，19眼，18眼，悪化はそれぞれ0眼，4眼，6眼であった。中心窩の病巣部の厚さは治療前が523μm，1か月後が517μm，3か月後が500μm，6か月後が428μmであった（p＝0.047）。4眼が再照射，4眼が他の治療を必要とした。結論：中心窩下脈絡膜新生血管に対する経瞳孔温熱療法の6か月後の評価で，70％以上の症例で視力が維持され病巣が退縮する傾向があった。
