近視学童における0.5％トロピカミド点眼による屈折値の変化

目的：トロピカミド点眼による屈折値の変化から近視学童の調節緊張を診断する可能性の検討。対象と方法：6～12歳の近視学童86名を対象とした。屈折の等価球面値は－1.25～－6.25Dであった。点眼には0.5％トロピカミドに0.5％塩酸フェニレフリンを加えた製品（ミドリンP^(R)）を用いた。屈折値は自覚的および赤外線自動レフラクトメータで測定した。結果：赤緑試験による自覚的屈折値は点眼前後で平均0.17±0.32D，自動レフラクトメータでは平均0.30±0.27Dの差で近視が軽減した。結論：ミドリンP^(R)点眼で調節緊張を診断するためには，自覚的屈折検査において，近視症例の95％信頼区間である0.8D以上の幅で近視が軽減していることが必要条件である。
