黄斑円孔術後に円孔底にトリアムシノロン残存を認めた1例

66歳女性が約10日前に自覚した左眼視力低下で受診した。矯正視力は右1.2，左0.1であり，左眼に黄斑円孔があった。その程度はGass分類で第2期に相当した。左眼に水晶体超音波乳化吸引術，眼内レンズ挿入術，硝子体切除術併用による手術を行った。硝子体を可視化するためにトリアムシノロンを硝子体腔に注入した。術中に円孔内へのトリアムシノロン迷入を認め，極力除去したが，一部が残存した。内境界膜剝離は行わなかった。術後，閉鎖した黄斑円孔底にトリアムシノロン顆粒が残存し，光干渉断層検査（OCT）でもこれに相当する所見があった。トリアムシノロン顆粒は術後3か月で観察できなくなった。術後5か月で左眼視力は0.9になり，術後1年の現在まで眼圧，眼底所見，視力などに問題は起こっていない。トリアムシノロンが黄斑下に残留しても，微量であれば無害であることを示す症例である。
